Kindergarten Registration is May 6, 2010.

WHO: Parents and guardians of children who will be five years old on or before Sept. 1, 2010.

WHAT: Kindergarten registration for the 2010-11 school year.

DATE: Thursday, May 6, 2010.

TIME: 7:30 a.m. to 2:00 p.m.

WHERE: B.B. Harris Elementary School

PLEASE BRING:
Your child;
An official document showing proof of birth date;
A Georgia certificate of immunization (State Form #3231 (see New Rules below));
Evidence of vision, hearing, and dental exams obtained in the past year (State Form #3300);
Proof of residency in the B.B. Harris attendance zone (Mortgage or lease and one utility bill);
Photo ID of parent and proof of custody or guardianship if not the birth parent;
If you are living with someone, a residency affidavit must be completed as well;
The child’s social security card.

Your child’s registration will be delayed if you do not bring the needed paperwork.

 

New Immunization Rules for Students Entering Kindergarten & 6th Grade

Students enrolling in Kindergarten or Sixth grade must have a GA  Certificate of Immunization, form 3231 that show the new requirements to enroll:

· 2 doses of the Varicella (Chicken Pox) vaccine or documentation of disease.

· 2 doses of the MMR (Measles, Mumps, Rubella) vaccine.

Make your appointment now with your health care provider and beat the back to school rush. Your class placement could be delayed if you are missing these requirements.

El dia de matricula para entrar al Kindergarten es el 6 de mayo del 2010

 

Para quien?:Para los padres y tutores de los niños que van a cumplir los cincos años de edad el 1 de septiembre del 2010.

Como?: Los niños se matriculan para el año escolar del 2010 al 2011

A que hora?: Jueves, 6 de mayo del 2010.

Tiempo?: 7:30 a.m. to 2:00 p.m.

Donde?: Escuela B.B. Harris

Para que?: Para preparar a su niño para asistir a su primer dia de escuela el 9 de agosto del 2010.

 

Que debe traer el dia de la matricula?: Traiga a su niño; Un documento oficial que compruebe la edad del niño; Un certificado de vacunas del estado de Georgia (State Form #3231); Un comprobante de que se le ha hecho un examen dental, de audicion y de la vista en el ultimo año (State Form #3300);Un comprobante de que vive en el area correspondiente a la escuela; Un comprobante de que usted tiene custodia o que es el tutor legal/apoderado del nino; presente una tarjeta de identificacion con foto; y el numero de seguro social del estudiante (Sin embargo los padres tendran que firmar una declaracion antes matricular al niño).

 

Si no presenta los documentos requeridos al momento de la matricula, esta no sera procesada.
