STEP 1: Please select the appropriate form:
Click here for Grades 1 - 5 Enrollment Form.
Click here for Kindergarten Enrollment Form.
Please Note: This DOES NOT complete the registration process. You will still need to come to school to provide the required documents and complete the registration.
STEP 2: Once you have completed the Enrollment Form, click here for a list of documents to bring to the school.
For students who live with their parents/guardians, but reside in the home of another adult, a NOTARIZED RESIDENTCY AFFIDAVIT is needed, in addition to the required documentation, as proof of residency of the person with whom the family is living. Click here to download the Residency Affidavit form, have it notarized, and bring it with you to your registration appointment.
Please be aware that your child will not be entered into our student database and assigned a teacher until ALL required documents are received. Please make copies ahead of time if possible.
STEP 3: Click on the link complete the Parent Portal Registration form. This will give you online access to your child's school information such as attendance, grades, etc.
